Health Assessment Form

Are you a MetroPlus Essential Plan member through the NY State of Health Marketplace? Make sure to fill out your Health Assessment Form (English / Espanol) and send it back to us. This form is intended to help determine any medical needs and provide us an understanding of what materials you may be interested in receiving.

Gym Reimbursement Form

All MetroPlus Marketplace or Essential members are eligible for gym reimbursements up to $200. Spouses are also eligible to receive an additional $100. A member must be active and complete 50 visits during six consecutive months of your coverage. For more details and to get the form, click here.

Pay my premium?

Pay My Premium Online

To make a secure online payment, remember to pay only with MetroPlus. Click here. All you need is your ACCOUNT or EXCHANGE NUMBER and your INVOICE NUMBER.

Pay My Premium Using a Check

Members that choose not to use online payment can send their Invoice Coupon with a check to the address listed.

Report Fraud, Waste or Abuse

MetroPlus is dedicated to helping prevent health care fraud and investigates all allegations of fraud, waste or abuse. Fraud includes member fraud, provider fraud, employee fraud and vendor fraud. 

Examples of fraud include:

  • Billing for services that were not rendered 
  • Billing for office visits that you did not go to 
  • Submitting multiple bills for the same date of service 
  • Offering money or goods to use a member identification card 
  • Sharing of member identification cards 
  • Providing false information in order to obtain insurance coverage
