Health Home Services for MetroPlus members
At MetroPlus Health Plan, we can help you stay healthy by providing the support you need to manage your health. MetroPlus partners with NYC Health + Hospitals' Health Home and other network Health Homes to provide Care Coordination and Linkage services. Health Home services are available for MetroPlus members in HIV Special Needs Plan (SNP), MetroPlus Enhanced, and Medicaid Managed Care.
What is a Health Home?
A Health Home is a care coordination service which enables all of an individual’s care givers to communicate with one another so that medical, behavioral and social needs are addressed in a comprehensive manner. As a MetroPlus member, Health Home services are 100% free of charge.
Health Home Services were created by the Affordable Care Act of 2010, Section 2703. The goal of Health Home is to coordinate care for people with Medicaid who have chronic conditions. The Health Home providers operate under a “whole-person” philosophy to integrate all primary, acute, behavioral health and long-term service and supports to treat the whole person.
Health Home Services:
- Comprehensive care management
- Care coordination
- Health promotion
- Comprehensive transitional care/follow up
- Patient and family support
- Assessment and referral for community and social services
Health Home Participation
Medicaid recipients suffering from chronic conditions are assigned to a Health Home.
Participation in a Health Home is entirely your decision. Refusal to participate has no negative consequences on your MetroPlus Health Plan membership and/or benefits.
The Health Home service is completely free of charge.
If you are currently receiving medical, mental health or substance abuse treatment you may continue to receive these services through your existing provider(s) and still benefit from the care coordination services provided by the Health Home.
