MetroPlus Medicaid Managed Care
MetroPlus Medicaid Managed Care is available to people who have or are eligible for Medicaid and live in Brooklyn, Manhattan, Queens or the Bronx. You may be eligible for Medicaid if you have high medical bills, receive Supplemental Security Income (SSI) or meet certain income, resource, age or disability requirements.
The chart below shows how much income you can receive in a month and qualify for MetroPlus Medicaid Managed Care. The income and resource levels depend on the number of your family members who live with you.

Some people are eligible for Medicaid who make more money than the chart shows. Pregnant women and children can have higher income levels and no resource limits. Expanded income levels for children and pregnant women are as follows:
• Infants to age 1 and pregnant women - 200% of the federal poverty level
• Children age 1 through 5 years - 133% of the federal poverty level
• Children age 6 through 18 years - 100% of the federal poverty level

Pregnant women, children, disabled persons and others may also be eligible for Medicaid if their income and/or resources are above these levels and they have medical bills. If an adult has too much income and/or resources and is not eligible for Medicaid, they may be eligible for Family Health Plus. If a child has too much income and is not eligible for Medicaid, the child may be eligible for Child Health Plus.

MetroPlus Medicaid Managed Care offers benefits such as:
• Regular check-ups and office visits with your own Primary Care Provider (PCP)
• Referrals to specialists in our network
• Live customer service representatives who speak your language
• 24-hour emergency hotline
• Well-baby and well-child care visits
• Family planning
• Vision and hearing examinations
• Lab tests and x-rays
• Hospital care
• Prescriptions
• Personal Care / Home Attendant Services
• Dental care provided using your Medicaid card
When you enroll in MetroPlus Medicaid Managed Care, you have a choice of at least three PCPs. If you want to continue to see your current doctor, make sure that your doctor is in the MetroPlus network.
