MetroPlus Select Plan (HMO)
To be eligible for MetroPlus Select Plan (HMO), you must have Medicare Parts A and B; reside in Manhattan, Brooklyn, Queens or the Bronx; and not have End-Stage Renal Disease (ESRD).
In 2010, MetroPlus Select Plan (HMO) provides all the benefits of Original Medicare PLUS:
• $26.50 plan premium. The amount you pay depends on your level of extra help.

• $0 copayments for Primary Care visits
• Medicare Prescription Drug Coverage (Medicare Part D)
• Up to $500 for hearing tests every two years
• Vision Services - One (1) pair of glasses every two years or one (1) pair of contacts
every two years
• Up to $400 a year in non-prescription drugs and health-related items using a MetroPlus issued card. Call MetroPlus Medicare Customer Services for details.
