MetroPlus Medicare Partnership In Care Plan (HMO)
To be eligible for MetroPlus Medicare Partnership In Care Plan (HMO), an HIV/Special Needs Plan, you must have Medicare Parts A and B; be HIV+; reside in Manhattan, Brooklyn, Queens or the Bronx; and not have End-Stage Renal Disease (ESRD).
In 2010, MetroPlus Medicare Partnership In Care Plan (HMO) provides all the benefits of Original Medicare PLUS:
• $26.50 plan premium. The amount you pay depends of your level of extra help.

• $0 copayments for Primary Care visits
• Medicare Prescription Drug Coverage (Medicare Part D)
• Hearing Services - $0 copayment for routine hearing exam once a year
• Vision Services - One (1) pair of glasses every two years or one (1) pair of contacts
every two years
• Up to $400 a year in non-prescription drugs and health-related items using a MetroPlus issued card. Call MetroPlus Medicare Customer Services for details.
• Up to $360 a year ($90 every three months) in reimbursable expenses for using taxi
and/or livery services
• Choose an HIV Specialist as a Primary Care Provider
• HIV Specific Health Care Screenings
• Specialized Care Management Programs, including AsthmaPlus and DiabetesCARE
(plus Telehealth Program)
• Coordination of medical and psychosocial services
