How to Renew
Medicaid Managed Care
1. Complete the renewal application you receive from NYC Human Resources Administration (HRA). If you lose your application or do not receive one, you may have a new one printed at certain sites. For more information, email MetroPlus or call MetroPlus at 800-475-METRO (800-475-6387) or TTY 800-881-2812 or HRA at 888-692-6116.
2. You return the application with updated proof of income via mail. If your information has changed, corrections must be made on the application and any required proof must be provided.
3. If you need help, call MetroPlus at 800-475-METRO (800-475-6387) or TTY 800-881-2812 or email us. MetroPlus renewal representatives can assist you on the phone or at a MetroPlus location in your community.
4. HRA sends you notification of reactivated status.
5. If your income changes making you ineligible for one product but eligible for another, you will be notified.
