How to Renew
Child Health Plus
1. Complete the renewal application you receive from MetroPlus by mail 90 days before your renewal date.
2. You return the application to MetroPlus with updated proof of income via mail. A premium check must be included with the renewal application, if applicable.
3. If your information has changed, corrections must be made on the application and any required proof must be provided or your enrollment may be delayed.
4. If you need help, call MetroPlus at 800-475-METRO (800-475-6387) or TTY 800-881-2812 or email us. MetroPlus renewal representatives can arrange to meet you at a MetroPlus office, your work location or home.
