Home > Members & Applicants  > Recertification Information IT'S IMPORTANT TO RECERTIFY/RENEW YOUR MEMBERSHIP. We want to keep you as our Member! As a member of MetroPlus Health Plan you need to recertify/renew your membership every year. If you are a Member of Child Health Plus B (CHP B): A MetroPlus Recertification Representative will contact you by mail and phone and send a pre-printed recertification/renewal application package to you. Contact our office to schedule an appointment or go directly to one of our sites for help with completing the recertification/renewal application. You can also call the MetroPlus' Recertification Line at 212-908-4190 if you have any questions or need help. If you are a Member of Medicaid or Family Health Plus (MA & FHP): Human Resources Administration (HRA) will mail a pre-printed application package to you about 60 days before your recertification/renewal date. You must complete the application and return it to HRA to recertify/renew your membership. If you have not received an application you can call the HealthStat phone line at 1-888-692-6116.
