The MetroPlus Marketing Department has representatives available to assist you in completing an application for enrollment throughout the four boroughs of New York City (Brooklyn, Queens, Manhattan and the Bronx). Representatives are available at the following hospitals and health centers.

In addition, you can walk-in to our office at 160 Water Street, 3rd Floor New York, NY 10038 from 9:00 am - 7:00 pm Monday - Friday and 10:00 am - 6:00 pm on Saturday to complete an enrollment application. We can also schedule a time to meet with you in your home or place of business to complete an application. You can call 800-475-METRO (800-475-6387) and speak to a Customer Service Representative to schedule an appointment.


You can also enroll in MetroPlus Medicaid Managed Care or Family Health Plus through New York Medicaid CHOICE, at 1-800-505-5678. New York Medicaid CHOICE can also tell you about Medicaid Managed Care and Family Health Plus in New York City, including what health plans are available where you live.
