
MetroMom
The MetroMom program can help a pregnant member choose a doctor for herself and her baby. The program
provides an expectant mother with support through her pregnancy and helps to plan care for her and her baby.
If there are any problems, the MetroMom Case Manager will arrange for services such as home care or other
specialty care. Other pluses include:
- Pregnancy Keepsake Calendars
- Family Planning
- Baby's First Year Keepsake Calendars
- Educational Materials
- Post-Partum Health Information

AsthmaPlus
The AsthmaPlus program is designed to help members manage their asthma. The AsthmaPlus nurse will
work with members and teach how to avoid things that bring on attacks and how to take medication properly.
AsthmaPlus program members will receive:
- The Asthma Action Plan - to help plan care
- The Peak Flow Meter - to help measure how well air flows in and out of the lungs
- The Spacer - to help get the most out of medications
- The booklet, "Living with Asthma" - with useful information about asthma

Diabetes
The Diabetes Care Management program is designed to help members with diabetes manage their blood sugar
levels. The Care Manager will help members schedule appointments and arrange for home care
and specialty services.

The MetroPlus Domestic Violence Program provides educational materials about preventing violence
and information about services that are available to help.
