There are several types of Care Management services available. These include:
- Care Management Programs
- Member outreach
- Member health and education materials
- Health education classes
- Provider health education seminars
MetroPlus Health Plan's Care Management Programs help members to:
- Promote wellness and improved health status
- Facilitate self management of health and disease states
They also:
- Advocate for members to ensure access to health and social services
- Encourage appropriate utilization
A Nurse Case Manager runs each of the Care Management Programs.
