MetroPlus Care Management Programs
At MetroPlus we can help you stay healthy and provide support to manage your health. We have special programs for people living with asthma, diabetes and HIV/AIDS. We also have programs for women expecting a baby, anyone dealing with family violence or mental health issues and members who have SSI or complex health problems. AsthmaPlus
Asthma can't be cured but it can be controlled. A case manager works with you and your Primary Care Provider (PCP) to manage your condition and make sure you have a current Asthma Action Plan. You need to know your asthma triggers and learn to avoid them as well as the correct way to take your medications.
DiabetesCare
If you have diabetes, you need to see your doctor every three to six months. Every three months, your A1C should be checked which tells how well your blood sugar is controlled. You need to work out a healthy food plan and stay with it. Your doctor will also check your weight, blood pressure and feet. Our Case Managers can work with you to figure out ways to reduce future complications and live a better life.
MetroPlus also offers a Telehealth program that provides the latest diabetic monitoring technology in your home and personalized nursing consultations over the phone.
MetroPlus Medicare
This program works with MetroPlus Medicare members and their PCPs to get all the services and referrals they need. MetroPlus Medicare members can also get help from our other care management programs.
MetroMom
This program helps pregnant women get the medical care they need for themselves and their babies. Our Maternal-Child Case Managers work with you through your pregnancy and can answer your questions. Special services like home care or transportation can be arranged. MetroMom members get a Pregnancy calendar and a Baby's First Year Calendar as well as educational materials.
Partnership in Care (for people living with HIV/AIDS)
If you are living with HIV/AIDS, this program helps you manage your condition to stay healthy. Our Case Managers help you make appointments with an HIV Specialist as your PCP and stay on top of your medications. We can also make it easier to see specialty providers such as Hepatitis C experts, dermatologists and heart specialists. For more information, see our Medicaid HIV Special Needs Plan.
Behavioral Health
This program is available to members who are stressed about life or managing their illness. Behavioral Case Managers can perform an assessment of your needs by telephone and work with you to find a Behavioral Health provider to treat you. Services are kept confidential.
Complex Case Management/Healthy Heart/Disabilities
This program is designed for members who may not meet criteria for other programs but have serious health problems that require help to access medical care. A Case Manager will complete an assessment by phone with you. Then, based on your needs, the Case Manager works with you and your doctors to coordinate the best possible care.
Domestic Violence
Violence should not happen to anybody, but it does. And when it does, there is help. Maybe you don't know who to talk to or have had a bad experience before when you tried to get help. We will listen to you without letting others know you called and get you the help you need. We can help you obtain resources to get you out of harm's way.
Smoking Cessation
Smoking can lead to many serious health problems, like lung cancer, stroke and heart disease. Quitting is one of the biggest steps you can take to improve your health and the health of those around you. If you smoke and want to quit, our Case Managers can help you.
Supplemental Security Income (SSI)
This program is for any member receiving SSI benefits who needs assistance with their medical conditions. Case Managers can assist you to understand your illness or medications, set up medical appointments, arrange transportation to appointments or help you obtain resources in your community.
